Peak Sports Psychiatry provides psychiatric consultation and mental health services tailored to athletes and athletic organizations. Services include direct patient care, collaborative care models, consultation services, and educational programming.
Our providers are licensed to practice in specific states. Services are only available to individuals located in states where our providers hold active licenses. Current licensing information is available upon request.
Scheduled appointments require adequate notice for cancellation or rescheduling. Specific cancellation policies will be provided when scheduling services. Repeated no-shows or late cancellations may result in termination of services.
Payment is expected at the time of service. We are an out-of-network provider for all insurance plans. We provide superbills (detailed receipts) that you can submit to your insurance company for potential reimbursement based on your out-of-network benefits. Patients are responsible for understanding their insurance coverage before scheduling services.
All content on this Site, including text, graphics, logos, images, audio clips, video clips, and software, is the property of Peak Sports Psychiatry or its content suppliers and is protected by United States and international copyright laws.

Any dispute arising from these Terms or your use of our services shall first be attempted to be resolved through good-faith negotiation. If negotiation fails, disputes shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association.
You agree that any dispute resolution proceedings will be conducted only on an individual basis and not in a class, consolidated, or representative action.

If any provision of these Terms is found to be unenforceable or invalid, that provision will be limited or eliminated to the minimum extent necessary so that these Terms will otherwise remain in full force and effect.
